I think it is more likely the doctor hit exactly the right spot, and it is possible on your previous shots that he/she was slightly off, although the botox does diffuse slightly in the muscle so close is often good enough.

I would suspect that if you get similar placement next time it would help if you had a lower dose. It's always a good idea to keep records of successive treatments - ask the doctor which cord(s) are being injected, the dose, how many sites in a cord, etc and document how long the breathy period lasts, and how long the good voice lasts, and so on. That kind of information helps you and your doctor discover the optimum treatment.

Having said all that, if excess breathiness continues to be a problem, you could consider asking for a lower dose in future, and/or the option of having one vocal cord only injected (this is termed a 'unilateral' injection. There is extensive discussion on unilateral injections in the archives.
