Edit
Don't give up on your job yet. I was also diagnosed in early 2007. I went through a year of "what do I do now" thinking. I never took a Botox shot. The idea of it just didn't sit well. I have my own business and so the thought of giving it up was not reasonable. I went through a special clinic and some pretty intense voice therapy. My earliest most successful moment was going through a drive through and ordering food. The plus side was I had stopped using drive throughs. However, the first place that you need to start is to look long and hard at your lifestyle and your personal health. Get off everything that is unhealthy including caffeine, alcohol, sugar, any allergy producing food. This isn't a forever type of diet but eliminating what might compromise your health even slightly might help your road to recovery. I've reintroduced some caffeine and sugar foods into my diet, but have eliminated alcohol to date. Consistent exercise also helps to get the breathing and anxiety removed. I also see a chiropractor once a week for neck and shoulder adjustments and massages. For me this didn't start overnight, so I figure that an overnight fix isn't available either. Find a good! voice therapist who knows SD and do it intensely as you can. Even if that means repeating the same c and h words over and over and over. Once day it just clicks and you'll have a constant smile on your face. Good luck and don't give up on your job. Maybe a temporary reassignment would take the pressure off and allow you to recover.
