If you've never been diagnosed and it's not in any medical records anywhere, then it shouldn't be considered a pre-existing condition. You may think you have it, but it takes a doctor to actually make the diagnosis. Allergan, the makers of Botox, expanded their Patient Assistance Program to provide Botox at no charge to financially eligible patients who are either uninsured or underinsured.
